The Care Finder (CF) program is an outreach service that targets people who are eligible for aged care services and who require intensive support to navigate the My Aged Care system and access to relevant supports in the community.
The worker will network with key stakeholders to gather eligible referrals, participate in the intake process and link clients into services.
The target population is marginalized, often with no carers or support person and may have one or more of the following characteristics:difficulty communicating because of language or literacy and/or find it difficult to understand information and make decisions and/or be reluctant to engage with aged care or government and/or be in an unsafe situation if they do not receive services.
-Ideally, you will possess:
- A professional qualification in Social Work.
- Experience in service coordination and community Aged Care.
- Demonstrated ability to comprehensive networking abilities, skilled referral processing and case planning.
- Excellent knowledge of Aged Care Sector and referral pathways.
- A clear understanding of working with marginalized communities and barriers they experience.
