About the role
The case manager is responsible for delivering person-centred case management to the residents of within Calvary Aged Care services.
You will work closely with residents, their families and relevant stakeholders to support in decisions around finding the best care solution within Calvary with a focus on privacy, dignity, choice and person centred care.
Key ResponsibilitiesProvide Case Management for the agreed program in the designated area utilising evidence based practice to deliver quality care and service, ensuring alignment with Aged Care standards
Assess resident needs, developing a collaborative, strengths-based accommodation Plan with the resident and their families and implementing services which will meet their individual needs.
Monitor and respond to all changes in resident requirements, according to agreed expectations, providing prompt feedback to relevant stakeholders.
Facilitate referrals and work collaboratively with the Project Team, Clinical Care team, Allied Health services and Medical Practitioners to address identified clinical needs
Undertake ongoing communication with Resident's and families, including review resident's accommodation Plan and goals at agreed intervals, as circumstances dictate and as indicated by the program guidelines

Demonstrated experience in the provision of case management services
Demonstrated experience in the aged/community/disability sector particularly working with individuals with complex needs.
Demonstrated commitment to the delivery of person centred, focussed care.
Highly developed communication skills to ensure that the resident's needs and areas of opportunity are identified and actioned as part of the Case Management process.
Highly developed proficiency using computers including the Microsoft Office Suite with the ability to learn and adapt to new databases.
