About the role

The TACS Social Worker is responsible and accountable for providing a professional level of service to the
Transitional Aged Care Service Calvary Health Care Kogarah (CHCK). The TACS Social Worker is expected to be a
competent clinical practitioner who is able to work under mínimal direct professional supervision and will function
as part of the multidisciplinary team.

Key Responsibilities

The Level 1-2 Social Worker is responsible for providing a professional level of service to the Transitional Aged
Care Service CHCK. They are required to undertake case management and social work assessments and plan and
implement treatment in line with relevant policies and guidelines.

The Level 1-2 Social Worker is responsible and accountable for providing a professional level of service to the
health facility. Deployment to other areas of CHCK may be also be required.

The Level 1-2 Social Worker will exercise professional judgement commensurate with their years of experience,
as experienced is gained, the level of professional judgement increases and direct professional supervision
decreases.

The Level 1-2 Social Worker will participate in quality activities and workplace education.

What you bring

Degree in Social Work and eligibility for membership of AASW.

Current Class C drivers licence.

Demonstrated skills in the provision of social work psychosocial assessment, intervention and case
management of older people with complex needs.

Experience working in Aged Care or Rehabilitation with a sound knowledge of community resources.

Well-developed time management skills, excellent written and verbal communication and a high level of
computer literacy.

Demonstrated ability to work with mínimal supervision and as part of a multidisciplinary team

Commitment to continuous service improvement, reflective practice and service development.

A desire to join a team committed to the care of others in a mission based organisation
