About the Role- Delivering high quality evidenced based Social Work clinical services to patients/consumers within a highly functioning interprofessional team.
- Contribution to the advancement of quality Social Work services and optimal patient care, through supporting quality improvement initiatives.
- Supporting patient and family participation in family meetings, case conferences, ward rounds and other relevant meetings.
- Referring to and liaising with other agencies/service providers.
- Supporting timely and safety discharge planning and coordination of patient care develop
- Focus on enhancing the patients social and emotional functioning through targeted specialised interventions and mobilising supports and services where required to assist in safe discharge planning.
