    National Care Society is seeking a dedicated, compassionate and out of the box thinking Support Coordinator to join our team in Cairns. In this role, you will be instrumental in providing critical support coordination services to individuals with diverse needs. Your primary responsibility will be to empower our clients, helping them to navigate and connect with various support services to enhance their quality of life.

    **Key Responsibilities**
    - Develop and maintain support plans tailored to individual needs.
    - Coordinate and manage access to required services, including healthcare, community activities, and educational programs.
    - Liaise with families, carers, and other stakeholders to ensure a holistic approach to client support.
    - Monitor and evaluate the effectiveness of support plans and make necessary adjustments.
    - Oversee and manage budgets to best utilise participant NDIS funding.

    **Qualifications**
    - Tertiary qualifications in Social Work, Psychology, Nursing, or a related field.
    - Proven experience in a similar role, preferably in the healthcare or social services sector.

    - Excellent communication and interpersonal skills.
    - Strong organisational and time-management abilities.
    - Empathy and the ability to work with individuals from diverse backgrounds.
    - A strong problem-solving mindset with a focus on client-centric solutions.
