Description

We are looking for a highly motivated Mental Health Worker/Group Facilitator with experience working with young people and their families to join our Reconnect Mental Health Service team.

The Role


The Reconnect Mental Health Service is an outreach, early intervention service supporting young people aged between years and their families, where there is a risk of homelessness and emerging mental health concerns for the young person.

We assist young people to improve their mental health and achieve their goals. The service is based at Centacare's Port Adelaide office.


The Mental Health Worker will work closely with the young person, their family, the school and community to facilitate the young person's integration into the family and education.


In this role you will:

  • Develop and cofacilitate groups as an early intervention measure providing psychoeducation and supporting young people to recognise and enhance their strengths and resilience.
  • Manage a case load of clients with mild to moderate mental health presentations, providing counselling and therapeutic interventions through a range of modalities including, but not limited to, trauma informed Cognitive Behavioural Therapy, Acceptance and Commitment Therapy, Motivational Interviewing and Exposure Therapy.
  • Provide referral and advocacy support to young people and their families.
  • Engage with, and develop, collaborative relationships with relevant key stakeholders.
  • Relate to young people in a manner that is youth friendly and developmentally appropriate.
This is a fixed term contracted opportunity until 30 June 2026.


To be successful in this role you will need:

  • A minimum of a Certificate 4 in Mental Health or equivalent.
  • Experience working with young people with emerging mental health concerns.
  • Valid Department of Human Services (DHS) Working with Children Check (previously known as 'Child-Related Employment Screening Clearance').
  • Valid Drivers Licence.

Desirable

  • Degree in relevant Human Services discipline.
  • Experience using Penelope electronic client management system and DSS Data Exchange.
  • Experience in facilitating groups for young people.
