What you'll be doing
The SCHN Integrated Care department is aligned with the NSW Integrated Care Strategy of NSW Ministry of Health.

The SCHN Integrated Care Department's vision is to inspire and enable all children and young people to live their best lives through connected coordinated and collaborative care across all health and social care systems.

The SCHN Integrated Care Manager's role will be to develop, monitor and reach predicted outcomes to the strategic Integrated Care Program (ICP).

They will be responsible for the implementation and management of this plan and be required to work in close collaboration with key stakeholders including SCHN Executive, Ministry of Health, and Agency for Clinical Innovation, Local Health Districts, Primary Health Networks and other external parties.

The department aims to achieve the Quadruple aims of Healthcare including:
Integrated SCHN services across primary, secondary and tertiary sectors and with key partners
Improved patient experience of the health system
Reduced waiting times for patients as they navigate the system
Improved health outcomes for patients and better quality of life
Reduced avoidable or unnecessary hospitalisations

  • Relevant tertiary qualifications and/or extensive relevant experience in health, social sciences or related discipline.
  • Broad knowledge of the health system in NSW and the national context.
  • Demonstrated experience in project and change management and the ability to meet timeframes and deliverables within budget. Preferred experience in research methodology for evaluation of programs.
  • Highly developed strategic, analytical and problem solving skills
  • Current unencumbered Drivers Licence for use within NSW/Australia.

All NSW Health workers are required to have completed a primary course (2 doses) of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA).

New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
