Full time permanent opportunity as a Support Coordinator to connect our customers with the services and support they need to live an empowered life.


  • Based in the Moreton Bay/ Brisbane North region + working from home (home location must be situated within Moreton Bay or Brisbane North area or surrounding suburbs as facetoface customer contact will be required)_

As our
Support Coordinator, you will work alongside customers with Multiple Sclerosis, Motor Neurone Disease, and other progressive neurological conditions to connect them with the services and support they need to live an empowered life.

Working from your own home office based in Moreton Bay/ Brisbane North region or surrounding suburbs, join our SPARK NeuroCare team to deliver adaptable, diverse, and accessible service for our customers.


Additionally, you will:


  • Partner with customers and their families to identify goals, needs and opportunities
  • Develop and maintain strong relationships across the sector to enhance customer choice
  • Provide NDIS Support Coordination and create opportunities to improve quality of life
  • Drive change and help grow a culture of collaboration, respect, and continuous improvement

About you:


To be successful in this position, you will want to make a difference in the lives of others, as well as effectively build rapport and connect with internal and external stakeholders.

Also, you will bring the following skills and experience:


  • Qualification or equivalent experience in disability or related human services fields
  • Highly developed and engaging written and verbal communication skills
  • Good Working knowledge of the NDIS and My Aged Care service environments
  • Networker able to navigate complex challenges within a community setting
  • Exceptional ability to selfmanage
  • Team player with a downtoearth, energetic approach

An NDWS clearance and completion of the NDIS Online Worker Training Module will be required for this role (prior to commencement).
