About the Role:
Program Worker - Upper Hunter (Singleton) location. Full time contract position until 30 June 2024.


The Social Futures Care Connect program is an aftercare service supporting people who have attempted suicide and focuses on empowering people to connect with both formal and practical supports to assist with recovery.

The Program Worker, Care Connect is part of small team that works with participants in the program, to increase access and engagement with care and supports and provides encouragement and follow up via outreach visits to prevent suicidal and self-harm behaviour.


Desired Skills and Experience:


  • Tertiary qualifications or equivalent experience in community services, health, psychology, social work, social science or social welfare, health promotion or similar
  • At least 2 years demonstrated experience in the delivery of services to people experiencing mental health issues and / or suicidality.
  • Experience in providing case management to vulnerable groups
  • Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people to ensure effective relationships
  • Demonstrated welldeveloped organisational, time management and administrative skills with the ability to plan, prioritise and meet deadlines including high quality reporting
  • Demonstrated experience in maintaining professional boundaries while engaging in personcentred work
  • Demonstrated ability to follow work processes including safe work practices
