As one of Australia's most innovative health and aged care providers, we aim to set the global standard of relationship-based care for people with complex needs. HammondCare provides care primarily through hospitals, residential care homes and home-based care as well as services for dementia support, community palliative care, rehabilitation, positive aging and older persons' mental health.

    About the role

This exciting new role sits within the HammondCare Foundation team. The Foundation exists to fund opportunities that would not otherwise exist for HammondCare patients, residents and clients who are experiencing disadvantage, and to extend HammondCare's model of care to reach more people who others won't or can't.

The Foundation has the privilege of translating HammondCare's Mission into action. And we want to do more.
