- Effective relationships are established with clients that encourage self-determination
- Trauma informed, strength based, and client centred practice principles are evidenced in all client supports
- Provide brief intervention, service coordination, case management, referral, and advocacy to successfully achieve client goals
- Case management support is provided to clients with complex needs
- Build connections with internal TSA Programs, services and streams
- Degree qualification in Social Work, Social Science, Psychology, or related field
- Relevant experience in the social services sector, including case management
- Able to demonstrate knowledge of homelessness, including context, causes, models, and approaches relating to best practice
- Experience working with internal and external stakeholders to both maintain and create new connections in the sector
- Current Australian Driver's License is required

About the role
We are seeking a dedicated Case Manager for our Green Light Supportive Housing Program You will provide assertive engagement, brief intervention, case management and service coordination with people who have experienced chronic homelessness.
About Greenlight Support Housing Program
Runs in partnership with Vincent Care and Sacred Heart Mission through a multi-disciplinary team including supportive housing workers, mental health clinicians, and peer support settlement workers. We support people making the transition from chronic homelessness and rough sleeping to placement in longer term housing to address their unique experience of homelessness and the underlying health and well-being factors to maintain their housing.
Responsibilities include
This is a permanent full-time position, based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Award Level 5.
