We are looking for a progressive and compassionate Case Manager who shares our mission & values and is committed to achieving positive outcomes.

The role you could play


We are seeking a Case Manager for our Women Outreach Support Service (WOSS) to provide short term case management support and assertive outreach to single women over the age of 20 (without accompanying dependants) who are experiencing homelessness, including rough sleeping.

You will work collaboratively with clients and other stakeholders to ensure the individual is provided with a transdisciplinary approach to support in their journey out of homelessness.

What is the Women Outreach Support Service?


This program provides short term interventions of support of up to twelve weeks to facilitate access to crisis and short term accommodation through the inner western metropolitan local government areas of Melbourne, Moonee Valley, Maribyrnong, Hobsons Bay and Brimbank.

The Case Management program provides person-centred and trauma informed support ensuring access for clients to relevant services, advocacy in obtaining safe and secure housing, specialised counselling, and psychiatric issues.

Duties

  • Trauma informed, strength based, and client centred practice principles are evidenced in all client supports
  • Provide brief intervention, floating support, case management, service coordination, referral, and advocacy to successfully achieve client goals
  • Case management support provided to clients in accordance with TSA case management models
  • Coordinate case conferences, planning and exit planning in conjunction with referring agencies
  • Clients assisted to access support services and establish community support
This is a permanent full-time position based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

  • Diploma or Degree qualifications in Social Work, Psychology, or related field required
  • Relevant experience in the Social Service sector including Case Management & Assertive Outreach practice
  • Experience working with internal and external stakeholders to maintain and create new connections in the sector
  • Able to demonstrate knowledge of homelessness
  • Resilience, flexibility and adaptability
  • Ability to meet tight deadlines
  • Advanced communication, written & organisational skills
  • Ability to ensure service delivery outcomes meet the required target
  • Ability to manage crisis situations
  • Initiative to solve service delivery challenges
