Case Manager Homelessness Services

A little about the role


The Case Manager position within the DPV Health Homelessness team works collaboratively with the Supported Referral program as part of the Northern Metropolitan Homelessness Network.

The position utilises practice skills within program guidelines and practice standards.

It adheres to continuous quality improvement processes for people in the program catchment who experience homelessness or are at risk of becoming homeless.

As a Case Manager within the Homelessness team, you'll support clients through:

  • Providing case management which adheres to organisational and state practice goals and standards;
  • Timely and appropriate assessment, engaging with consumers, intervention including outreach support, care plan coordination, identification of needs, assessing resources, supported referral, advocacy, information and disengaging and discharge of consumers; and
  • Developing care plans in conjunction with consumers and other relevant participants that identify goals and reflect a client selfmanagement approach.
This is a full-time 12-month parental leave contract based in Epping.


Desired Skills and Experience:


About you


To be successful in this role, you will have demonstrated understanding and case management experience within the homeless sector or other relevant community settings.


You will also come with:

  • Relevant Australian tertiary qualification in social work, community services, welfare studies or other equivalent and related discipline; and
  • Demonstrated knowledge of the Specialist Homelessness Services (SHS), Supported Accommodation Assistance Program, the homelessness service system and current developments including the DHHS Standards, Continuous Quality improvement and the Opening Doors Framework.
