The Home Care Packages Program provides older people who want to stay at home with access to a range of ongoing personal services, support services and clinical care that help them to manage their day-to-day activities.
We are looking for a Registered Nurse, Social Worker or Allied Health Professional, with management experience and a passion for aged care, to lead the care advisors and service coordinators in our Home Care Packages Team.
The Home Care Packages Coordinator is responsible for the operational management of the Home Care Package program and will be involved in the strategic planning and development of our Support at Home Services.
Ideally, you will possess:
- A professional qualification in Nursing, Social Work, or Allied Health.
- Experience in the delivery of home based care or Home Care Packages.
- Demonstrated ability to foster positive relationships with clients, colleagues and external organisations.
- Excellent knowledge of Aged Care Sector Reform.
- A clear understanding of Clinical governance and quality in aged care.
