About the facility


Barwon Health's Advance Care Planning (ACP) program consists of an active, enthusiastic, flexible and innovative team with a focus on person centred care, shared decision making, safety and quality improvement.

The program has received a number of quality awards for innovative projects which improve the overall service for our consumers.

We have had opportunities to present our work at local, national and international fora.


About the role


The ACP Program Team Lead will work closely with the program manager and team, oversee the day-to-day operations of ACP services and deputise for the program manager as required.


The role includes providing leadership to ACP program staff, triaging and managing the flow of patients and the day-to-day service requirements, including activities related to training, education, quality improvement and public awareness initiatives.

The ACP Program Team Lead will provide an expert resource for ACP program staff and across the organisation.


It is an expectation that this position will provide leave cover for the Barwon South West Region VAD Care Navigator, with the appropriate training.
