    Description

    A rewarding opportunity at our Avondale Heights Residential Aged Care Home for an experienced Clinical Care Coordinator. This is a full time position Monday to Friday

    The role will be responsible for the delivery of clinical care services including care planning, review and assessments.

    What you will bring:

    • Dedication to resident care and support
    • Excellent clinical knowledge regarding medical conditions, palliative care, and care of people living with dementia including managing challenging behaviours
    • High level interpersonal and written skills
    • Previous experience as a Clinical Manager
    • Demonstrated experience in managing staff, including performance management
    • Ability to effectively communicate with residents, resident representatives and other health professionals
    • A good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to clinical care
    • Current AHPRA Registered Nurse DIV 1 registration with no conditions
