About the role

This is a newly created role supporting our clients at The Christian Brothers facilities and homes.

The Care Manager is responsible for providing active case management and complex care coordination for brothers who are experiencing multiple medical conditions, with the aim of actively promoting quality of life, and good independent management of their chronic health issues.

About you

You are an Enrolled Nurse with a valid APHRA registration. You are a vibrant personality who enjoys working autonomously and has strong communication skills. You love the unexpected and take joy from solving a problem. We're constantly adapting and improving, so you're comfortable with juggling multiple priorities at once.

In addition to sound customer service skills, you will also possess the following:
Knowledge of medication management, including experience with Webster pack management
Wound management skills - experience in changing basic dressings and prescribing simple dressing regimes
Basic health assessment skills
Valid Victorian Driver's Licence and own vehicle

Case management or complex care coordination experience will be highly regarded.
