Western health are looking for an experienced Registered Nurse, Care Coordinator, who is passionate about integrated care and patient flow and has experience in complex discharge planning, to join our multidisciplinary custodial health team, that is responsible for the provision of primary healthcare at Dame Phyllis Frost Centre (DPFC), a maximum security women's prison.

On 1 July, 2023 the delivery of primary healthcare will transition from the current service provider to Western Health. The multidisciplinary team will provide holistic and interconnected service provision to prisoner's at DPFC.

Successful applicants will be on boarded during the transition period for orientation and training including additional training to support the delivery of culturally appropriate, trauma informed, gender affirming care for all patients including priority cohorts.

To be Successful in this Role you will

  • Have an appropriate qualification in nursing and eligibility for registration with relevant body
  • Possess at least 5 years acute clinical experience
  • Have experience in assessment of people with complex needs, discharge planning and care planning
