Full Job Description:


We are working with a respected aged care services organisation who are looking for a Permanent Full-Time Care Manager to join their Sunshine Coast office in a community-home-based full-time permanent opportunity.


In the Care Manager position, you'll use your aged care management, customer service and administration skills to make an impact on your clients and their needs.


A typical day includes:

  • Coordinating resident care needs
  • Managing care plans through judgment, creativity, and sensitivity to develop care plans that are aligned to the client's assessed needs
  • Develop, monitor, reassess, and documentation, HCP, CHSP care plans in consultation with clients, their families, and other allied healthcare professionals
  • Work in collaboration with clients and their families manage to facilitate and develop service offerings.
  • Manage and assist with incidents and complaints.
You will be part of a multi-disciplinary team that work collaboratively with ongoing support from management.


About you
***Building relationships and trust amongst your customer, you will have a genuine interest and passion towards aged care and putting aging Australians and their families at the heart of what you do.


You will

  • Hold a current Police Clearance (or be willing to obtain)
  • Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired
  • Frontline management skills
  • Current First Aid Certificate and current CPR certificate
  • The right to work in Australia
  • A current driver license, registration, and access to your own car with comprehensive car insurance
