Full Job Description:

We are working with a respected aged care services organisation who are looking for a Permanent Full-Time Care Manager to join their Sunshine Coast office in a community-home-based full-time permanent opportunity.

In the Care Manager position, you'll use your aged care management, customer service and administration skills to make an impact on your clients and their needs.

A typical day includes:

Coordinating resident care needs
Managing care plans through judgment, creativity, and sensitivity to develop care plans that are aligned to the client's assessed needs
Develop, monitor, reassess, and documentation, HCP, CHSP care plans in consultation with clients, their families, and other allied healthcare professionals
Work in collaboration with clients and their families manage to facilitate and develop service offerings.
Manage and assist with incidents and complaints.

You will be part of a multi-disciplinary team that work collaboratively with ongoing support from management.

About you
***Building relationships and trust amongst your customer, you will have a genuine interest and passion towards aged care and putting aging Australians and their families at the heart of what you do.

You will

Hold a current Police Clearance (or be willing to obtain)
Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired
Frontline management skills
Current First Aid Certificate and current CPR certificate
The right to work in Australia
A current driver license, registration, and access to your own car with comprehensive car insurance
