The Role

Attend and schedule in home visits to access care needs.
Coordinate care plans through consultation and sensitivity to develop plans that are aligned to the client's assessed needs.
Develop tailored care plans in consultation with clients, their families, and other healthcare professionals.
Work in collaboration with clinical care managers and families to manage to facilitate and develop service offerings.

About you

Experience within health care, aged care or a medical or insurance industry preferred
Empathic and caring nature with the ability to build strong relationships
A full Australian drivers license and access to your own car
