We are looking for a dedicated Home Care Coordinator to join our friendly Home Care team supporting the Northern Region within our Home Care portfolio.


About the role
The Home Care Coordinator is responsible for supporting person-centred & responsive services to our clients.

This role is the central point of call for clients within the Northern region ensuring the smooth delivery of Home Care services.


In this role you will assist with client onboarding, reviews, assessments and developing budgets for our clients ensuring they have a holistic assessment and service plan with a health promoting focus.

You will complete all relevant administration tasks associated with this process including all tasks associated with service coordination.


You'll work amongst a multi-disciplinary team to provide integrated care that promotes healthy ageing and wellbeing, social engagement, prevents frailty and avoidable decline and achieves reablement for clients to optimise their ability to live better in their own home.

About you
We are looking for people who will be the difference in the lives of our residents and clients.

  • Demonstrated experience in a client support role working with a diverse range of clients/groups.
  • Ability to demonstrate integrity, calmness and personal resilience during periods of adversity and pressure.
  • Be able to seek creative solutions to overcome any barriers.
  • Effective decision making ability, including the ability to identify potential risks and escalate as appropriate within the organisation.
  • Excellent customer service, communication and time management skills.
  • Passionate about personcentred care & enabling clients to maintain maximum independence.
  • A current and unrestricted driver's licence.
  • A Diploma of Nursing with current AHPRA registration as an Enrolled Nurse is desirable.
