About the Role
Our Wellness Partners (HCP Care Coordinators) coordinate, guide & oversee service provision (and information) to our Home Care customers.
You'll be helping them make decisions, identify and arrange resources that meet their identified care needs.
We'll also be asking you to oversee day to day management of funds / budgets.
This is a mobile and office-based role, with the majority of your time spent in the community visiting clients in your care portfolio.
Your days will be spent:
- Care coordination inclusive of nursing care and care plans to reflect the needs and goals of our customers using a planned case management approach
- Operate as part of a multidisciplinary team, promoting and using a consultative approach
- Customer service including building and maintaining relationships with existing and prospective clients, families and other relevant stakeholders e.g allied health team. Ensuring our customers receive the highest quality of clinical care to achieve their goals
- Liaising with preferred providers in relation to assessments, needs, requirements, and recommendations for products, treatment and services
- Maintain regular and clear communication allowing customers to keep track of their progress and progression towards agreed goals
- Promote quality and costeffective interventions and outcomes
About You
With a well-developed knowledge of the aged care sector, you have experience in a coordination / case management role. AHPRA Registered Enrolled Nurse.
As a natural problem solver, you can work autonomously and also work effectively as part of a multidisciplinary team.
You're comfortable managing complex clients who are receiving Home Care packages.
**To be considered for this role, you'll need: - **- Experience in aged care or community services.
- Familiarity with home care packages.
- Great interpersonal & communication skills.
- Ability to quickly learn & use new software for record keeping.
