We are looking for an experienced Clinical Care Manager to help up deliver high quality and meaningful care to our aged care clients.

So, what does the role entail?


Being a Registered Nurse, as Clinical Care Manager you will provide clinical guidance and clinical nursing services to clients through assessment and establishment of treatment plans in consultation with the client, within their own environment, with a focus on wellness and re-ablement,.

Your expertise and actions will allow the client to maintain their independence for as long as possible. Your daily responsibilities will include;

  • Ensuring care is delivered in accordance with your scope of practice and registration with the Nursing and Midwifery Board of Australia and Australian Health Practitioner Regulation Agency
  • Delivering excellent client care for elderly Australians.
  • Be a leader in the field for the myHomecare Group.
  • Managing and overseeing a high standard of care provided by the myHomecare Group providers and staff.
  • Using your customer service and aged care management skills to make a mark on the community. In doing so, you'll help keep aging Australians safe at home for longer. Crucially, your contribution will help to maintain the myHomecare Group's leadership in quality homecare.
  • Developing, monitoring, and reassessing care plans in consultation with clients, their families and other healthcare professionals.
  • Managing and developing a team including the Assistant Care Manager and support staff.
  • Working in collaboration with our Nurses, Regional and other Managers to facilitate, develop and shape our service offerings. Critically, you'll ensure our services continue to be of the highest standard.
  • Monitoring the budget of care packages and ensure clients understand service fees.
  • Managing client service delivery across all levels under the government funded CHSP and homecare packages programs.
  • Engaging with internal teams to facilitate client onboarding through the lead to conversion process.
  • Utilising you're influencing and partnering skills to lead and work collaboratively with our schedulers, clinical team and support staff.
  • Ensuring compliance with all relevant legislative and industry standards.
  • Proactively engaging in continuous improvement across practice and service delivery.

  • AHPRA Registered
  • Registered Nurse
  • Experience in aged care with knowledge of Home Care Packages, Commonwealth Home Support Program and Aged Care Quality Standards.
  • Frontline management skills.
  • Current First Aid Certificate and CPR certificates.
  • The right to work in Australia.
