    selectability provides and coordinate NDIS supports, aged care services, clinical services and deliver government funded mental wellbeing, carer and youth support programs.

    About the role

    selectability are seeking a dedicated and experienced Case Manager to spearhead our Transition Care Program with a specific focus on aged care. The successful candidate will play a pivotal role in ensuring the seamless coordination and delivery of therapeutic care plans, discharge planning, and support services to clients and their families.

    Main duties:

    • Lead and oversee the Transition Care Program with a strong emphasis on aged care, nursing duties will be required.
    • Collaborate with interdisciplinary teams to ensure the effective implementation of program objectives.
    • Provide guidance and supervision to program staff to ensure the highest standards of care and service delivery.
    • Develop and implement comprehensive therapeutic care plans for clients, considering their unique needs and circumstances.
    • Conduct consultations with community services to enhance collaboration and support networks for clients.
    • Maintain accurate and up-to-date documentation and clinical records in accordance with regulatory standards.
    • Facilitate smooth and well-coordinated discharge planning in collaboration with healthcare professionals, ensuring continuity of care for clients post-transition.
    • Provide support to clients and their families to address the emotional aspects of the transition period.
    • Deliver education sessions to clients and families, promoting health literacy and empowering them to actively participate in their care.
    • Offer ongoing support services to enhance the overall well-being of clients throughout the transition period.

    About You

    To be considered for this rewarding role you will have:

    • AHPRA registration (Nurse or health practitioner)
    • Previous experience in program leadership and case management
    • Strong understanding of therapeutic care planning and discharge planning processes
    • Ability to work collaboratively in a multidisciplinary team
    • High organised, time management and excellent written and verbal communication skills
    • Excellent written and verbal communication skills

    You will also need:

    • a current minimum P1 "C" class Queensland drivers licence
    • a currently registered, roadworthy, and fully comprehensive insured vehicle
    • a Positive Notice Working with Children (blue card) (or the ability to obtain prior to commencement)
    • a current yellow card exemption or an NDIS Worker Screening Clearance (or the ability to obtain prior to commencement)
    • NDIS Worker Orientation Module 'Quality, Safety & You' completion certificate (visit to access this free module). at completion of the course, you will be provided with a certificate of completion which you can forward to

    About the offer

    This is a full-time role based in Townsville, with the flexibility of Part Time or Casual hours for the right candidate.
