ROLE OVERVIEW:


KEY RESPONSIBILITIES:


  • Assess the needs of individuals with disabilities to develop personalised care and support plans.
  • Coordinate and oversee the delivery of a range of services, including therapeutic support, community participation, household tasks, and travel/transport assistance.
  • Actively engage with participants and their families to ensure services are delivered in alignment with their goals and preferences.
  • Work collaboratively with healthcare professionals, support workers, and other stakeholders to achieve optimal outcomes for participants.
  • Monitor and review the effectiveness of support services, making adjustments as necessary to meet changing needs.
  • Facilitate innovative community participation initiatives to encourage social connection and active engagement of participants in the community.
  • Ensure compliance with all relevant regulations and standards pertaining to disability care and support.
  • Provide ongoing support and advice to participants, empowering them to achieve greater independence and quality of life.

Requirements:


  • Australian Citizens and Permanent residents are encouraged to apply.
  • Minimum Diploma in social work, community services, disability services, or a related field.
  • Minimum 23 years of experience in disability care, community services, or a similar role.
  • Strong understanding of the challenges faced by individuals with disabilities and a passion for enhancing their quality of life.
  • Excellent interpersonal and communication skills, with the ability to work effectively with participants, families, and team members.
  • Demonstrated ability to develop and implement effective care and support plans.
  • Commitment to upholding the values and mission of Westview Disability Care.
