An exciting opportunity exists to join the The Heart Centre for Children at the Sydney Children's Hospital Network.

It is one of the largest and busiest sub-specialty teams within the SCHN and provides cardiac care to children of New South Wales and the surrounding regions at the Children's Hospital Westmead, Sydney Children's Hospital Randwick and within our outreach centres.

The Paediatric Heart Failure, Transplant and VAD CNC2 will work collaboratively with the Cardiac Services team to provide safe, compassionate, person-centred care to children and families.

Part time applicants or job share arrangements will be considered.


The CNC2 is expected to:

  • Undertake primary case management of children and families with heart failure and particularly postVAD implant and transplant patients, both inpatient and community based ensuring the safe and appropriate escalation of patient care
  • Coordinate the assessment, care and management of paediatric and adolescent patients requiring a VAD
  • Participate in the provision of rostered oncall services for the coordination of donor referrals and transplant coordination for heart recipients
  • Collaborate with all members of the multidisciplinary team and contribute to interdisciplinary clinical decision making including appropriate referrals to members of the multidisciplinary team and other stakeholders as required
  • Work with the team to assesses patient's readiness for transition to adult services and assist in preparation of a comprehensive nursing plan for adult coordinators and allied health staff
  • Update and disseminate individual patient care protocols for VAD patients and heart transplant recipients on the waiting list to relevant stakeholder's
  • Coordinate VAD surgical equipment allocation and clinically efficient utilisation of available resources.
  • Act as resource for multidisciplinary staff regarding postoperative care of VAD implantation and or cardiac transplant patients, including preparation of individual management plans
  • Support and lead review of nursing practices and health care delivery systems related to the management of patients with advanced heart failure, implementing changes to ensure that current practice reflects knowledge, research and best practice and outcomes
  • Initiates, conducts, and disseminates the findings of research in heart failure, transplant, VAD, pulmonary hypertension including presentation at conferences and leading and contributing to publications
  • Undertake primary responsibility for the planning and implementation of specialist clinical education and resources to clinicians across disciplines caring for patients with heart failure and VAD, including competency assessment
  • Identify the learning needs of patients and their families, and educate caregivers to acquire the information and gain the skills needed to care for their child throughout the transplant and/or VAD continuum
  • Contributes to the development and evaluation of evidence based clinical guidelines pertaining to heart failure and VAD care
  • Central source of communication regarding VAD patients for other SCHN departments and external agencies
  • Actively participates on relevant national and local committees, working parties, interdisciplinary and interdepartmental forums and develop and maintain collaborative relationships within and outside the organisation

What you'll be doing


The Clinical Nurse / Midwife Consultant ( CNC/CMC) will provide expert clinical care and education to children and their families within a defined specialty.

The role provides a consultancy and educational focus for ward based nurses in the care of children and their families.

In collaboration and consultation with key stakeholders, the CNC/CMC will undertake primary responsibility for on-going education, development and evaluation of clinical practice.


  • Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a minimum of 5 years full time post qualification experience, with at least three years full time equivalent experience in the specialty field.
  • Relevant approved post registration qualification in field of practice at Master's level or working towards or such other qualifications or experience deemed appropriate by the public hospital or public health organisation
