    Description

    We have a wonderful opportunity for an experienced and motivated Clinical Care Coordinator t o lead and mentor our dedicated team at our Residential Home in Hobart, Tasmania.

    The Clinical Care Coordinator plays a crucial role in coordinating person-centered clinical care for residents, adhering to established policies, procedures, and professional standards. Working in collaboration with the Care Manager, the primary purpose is to concentrate on individual resident needs. This involves ensuring the existence of a current and well-documented Care Plan that effectively addresses residents' needs.

    Your key responsibilities

    • Role Model: Acts as a role model for the organisation and contributes to the efficient and effective management of staff, material and fiscal resources according to the strategic plan and operational budget, this involves remaining on 24-hour call in the case of emergencies. It also involves having strong knowledge of all consumer health conditions by attending regular hand-over, being able to complete care plans and their reviews.
    • Rostering: Ensure appropriate knowledge and skill mix is maintained on each shift. Monitor roster as required to ensure there are adequate staffing levels and replacement of staff occurs including for sick leave and annual leave to ensure consumers are receiving optimal care.
    • KPIs: Complete monthly CCM component of KPIs including comments and actions and monthly Clinical Quality Indicator. Ensure actions from previous month's KPI is addressed and closed.
    • Clinical Care: - Coordinates the completion of clinical care tasks and related clinical documentation
    • Communication: Coordinate and liaise with consumers and/or their representatives, and all other health care professionals
    • Staff competencies and education: Coordinate staff competencies, monitor new staff appointments, provide education, performance management.
    • Quality Audit: Check and action gaps

    Your skills & experience

    • Bachelor of Nursing
    • Current Australian Health Practitioner Regulation Agency (AHPRA) registration
    • Nursing experience within a residential aged care facility, preferably at least 3 years of aged care experience.
    • Experience of working cooperatively with multi-disciplinary care teams
    • Strong clinical assessment, documentation and practice skills
    • Effective communication and interpersonal skills
    • Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
    • Experience in a similar clinical care role
    • An understanding of the aged care accreditation process
    • Working knowledge of quality improvement programs
