As the Aged Care Coordinator, you will play a pivotal role in overseeing and optimizing the care services provided to our elderly residents


Your responsibilities will include:

• Conducting comprehensive assessments of residents' needs and preferences to create individualized care plans.
• Coordinating a multidisciplinary team of healthcare professionals to ensure seamless care delivery.
• Monitoring the wellbeing and health status of residents, proactively addressing any concerns.
• Collaborating with families and residents to ensure their preferences and choices are respected and integrated into their care plans.
• Implementing quality assurance measures to maintain high standards of care and adherence to regulatory guidelines.
• Organizing and facilitating care conferences and meetings to review resident progress and care plan effectiveness.
• Providing support and guidance to care staff to ensure residents receive compassionate and personcentered care.
• Managing resources effectively to meet the needs of residents and maintain operational efficiency.

Qualifications:

• Bachelor's degree in Nursing, Aged Care Management, Health Services, or a related field. A Master's degree is a plus.
• Proven experience in aged care coordination or a similar healthcare management role.
• Excellent communication and interpersonal skills.
• Strong leadership and decisionmaking abilities.
• Knowledge of aged care regulations and quality standards.
• A genuine passion for caring for the elderly and improving their quality of life.
