    Right at Home is seeking a motivated and experienced Care Coordinator to join our team and play a crucial role in providing high- quality in home care services to our valued clients.

    Qualifications

    As a Care Coordinator, you will:

    • Recruit, train and supervise a team of carers to ensure the delivery of exceptional care services.
    • Coordinate and schedule operational activities in accordance with established care plans and organizational policies.
    • Liase between clients, families, referral resources, and administrative staff to facilitate effective communication.
    • Build strong relationships with clients and their families ensuring a positive and professional experience.
    • Educate clients and families on available services and benefits of care plans.
    • Monitor and assess the quality of care provided, identifying areas of improvement and implementing changes as needed.
    • Assist with inside sales enquires and contribute to business development opportunities.
    • Ensure compliance with regulatory agencies and safety standards.

    Preferred Skills

    • Certificate lll in Aged Care/Individual support/Community care (or higher qualification in relevant discipline)
    • Knowledge of Aged Care industry and related issues
    • Proven ability to assess, plan, implement and evaluate services.
    • Strong interpersonal and communication skills, both written and oral
    • Computer literacy and Analytical abilities.
    • Comittment to the Aged Care industry and the wellbeing of seniors and vulnerable individuals.
    • Driver's license and Comprehensive insured private vehicle.
