Specifically, we are looking for applicants with tertiary qualifications in a health related field and/or relevant health experience, working in chronic care services and/or community services and a NSW Drivers Licence.

Someone with demonstrated understanding of the provision of health care for people with chronic disease conditions and/or complex health needs including social care.

The Justice Health NSW Integrated Care Service (ICS) supports the existing systems to provide and integrated model of care for the patient journey from reception, through custody and into the community.

The ICS Care Coordinator position is responsible for the coordination of care for adult patients with chronic and/or complex health conditions.

The position provides a support network to the Network clinicians to assist with the continuity of care, case management, transfer of care into the community and post release follow up as required for custodial patients.

The role and responsibilities are to be carried out in a manner that is consistent with delegations, policies, procedures and operations systems of JH&FMHN, and in line with both the NSW Health Code of Conduct and JH&FMHN Code of Conduct.

  • Demonstrated creativity and innovation in approach to complex problem solving with the ability to negotiate, network, multitask and manage competing deadlines;
  • Demonstrated understanding of the NSW Health Integrated Care Strategy and a commitment to the values of the Justice Health and Forensic Mental Health Network.
