Your new team:

CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. We are seeking applicants for a permanent, full-time Case Manager.

This role is a unique opportunity to have a real impact in meeting the needs of one of our community's most complex and vulnerable cohorts.

You will form solid working-relationships within the pre-existing community teams, and foster strong therapeutic relationships with your clients. We support rehabilitation goals through the use of best evidence-based frameworks and assertive case management.


Your contribution:

You are a team player with a commitment to quality client care for people's living with mental illness:

You will have a professional qualification with relevant AHPRA Registration:

-
Minimum 3 years' experience in a relevant mental health setting (include acute psychiatry):

Demonstrated knowledge of recovery and collaborative clinical practice

A working knowledge of the Mental Health Act (2014), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines
