We are on the lookout for dedicated and passionate team members with experience working with people with disability and/or those in the aged care sector.

You will hold a case load of both NDIS and Aged Care package recipients and provide support to build capacity to maintain independence.

This is a full-time role Monday-Friday. This role will support the General Manager in a range of day to day administrative and support tasks.

Being the first point of contact for clients, families, and staff, you will be responsible for managing direct services to Care Service clients under a range of funding arrangements including but not limited to My Aged Care and NDIS funding.

Services are to be Client Driven at all times with quality outcomes for people that are achieved using a flexible, creative, and collaborative approach.

WHAT WE ARE LOOKING FOR:


  • Sound experience of the knowledge and skills required in working with frail older people, people with disability and their carers.
  • Welldeveloped interpersonal and communication skills, including the ability to initiate and maintain contacts with a range of service providers and community agencies.
  • A sensitive approach to working with individuals and a philosophy of empowerment.
  • Knowledge of the case management approach to service delivery in a rural setting with the ability to work as part of a multidisciplinary team.

KEY RESPONSIBILITIES, ACTIVITIES AND DUTIES:


This position forms part of the Case Management/Planning team responsible for providing Planning and Case Management services to our consumers and reports to the Service or Program Manager.

Case Management/Planning is predominantly a technical role confined to:
Care Planning, Assessment, Goal setting, monitoring effectiveness and formal reviews.


The key activities are as follows:

  • Operating in accordance with funding guidelines, funding and service agreements, program policies and procedures and associated legislation.
  • Establishing a new consumer on NDIS/Age Care services.
  • Developing a service agreement specific to client needs and a service delivery plan.
  • To liaise with NDIS/Age care representatives and other service providers as required
  • Monitoring the progress of clients against goals and assessing the adequacy of services provided.
  • Engaging in program administrative duties, including preparation of 8 week and 9month reports.
  • Compiling data concerning recipient and ensuring that services meet the needs and goals of recipients and reporting to the NDIS/age care when required.
  • To provide information to clients and their carers regarding the range and types of services available, allowing them to make their own choices.
  • To negotiate the access to appropriate services utilising lateral thinking and creative problem solving where necessary.
  • Lead excellence in service provision in Core Supports, Support Coordination, Community Access, Supported Independent Living, MTA, STA and SDA engagement.
  • Advocate on behalf of individuals in support of their rights and ensure that individuals are aware of advocacy agencies that they may be able to access.
  • Ensure appropriate negotiation and clarification of roles and responsibilities of all involved in working with an individual and their primary carer/s.
  • To provide flexible, innovative, welldesigned, and coordinated individual care plan that continues to meet the goals and needs of the individual and their carer.
