Your Opportunity


We have an opportunity for a Case Manager to join our TEI Child, Youth and Family Service based in Campbelltown and working across Claymore and Liverpool areas.

This service helps disadvantaged individuals and families by offering advice, support & referrals, case management, parenting and practical skill development groups.

This role is being offered on a part time, maximum term basis through to 30th June 2025, your primary working days will be Wednesday to Friday, however there will be some training needs that require flexibility in your working days during the first few weeks of employment.


Your key responsibilities will be to:

  • Respond to and ensure that new referrals meet the service criteria, have a clear understanding of how the service operates and consent to participate in data collection
  • Work with clients to create individual client driven case plans including referral to supplementary services as needed
  • Provide ongoing case management with participants and review progression against case plans
  • Assist participants in the process of transition out of the service into independence or other services including the support team
  • Develop quality relationships with a range of key stakeholders in Campbelltown and Liverpool including community organisations, local schools and relevant health professionals to allow for the effective provision of support and information to clients
  • Develop and conduct a range of groupwork programs for clients on a range of topics relevant to adolescents and families
  • Conduct client risk assessments and liaise with Service Manager to ensure that work is undertaken in a safe manner

Requirements for Success

  • Previous experience with Case Management and working to a Case Plan
  • Tertiary Qualifications in Social Work/Youth Work or relevant related discipline
  • Detailed knowledge of youth issues relating to, mental illness, alcohol and drug use etc
  • Excellent written and oral communications skills, including ability to communicate effectively with clients, team members and other service providers
  • Current and valid Drivers Licence


Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
