About the role

A rewarding opportunity for a passionate Family Liaison Officer to join the Dementia Support Australia (DSA) team at HammondCare on a
Fixed Term Full-time contract until January 2024 at our
Horsley site.


The Family Liaison Officer works within the Mission and Mission in Action of HammondCare to reduce emotional distress and enhance and promote the wellbeing of clients and their families who engage with the Acute Residential Care Transition Service (ARCTS).

The role aims to reduce emotional distress and promote the wellbeing of the clients and their families.


Responsibilities

  • Provide pre and post support and counselling to families.
  • Collection of data to better understand what is required for successful placement of a client living with dementia from an acute setting into a Residential Aged Care Facility (RACF)
  • Contribute to the assessment of clients' needs, abilities and behaviours and contribute to the individual client care planning.
  • Facilitate negotiation between families, Acute Facilities and Aged Care facilities during the pre and post placement phase.
  • Build rapport and a working relationship with families participating in the program.
  • Understand the opportunities and challenges around the transition and placement of someone living with dementia.
  • Contribute to assessing clients' needs, abilities or behaviour using a variety of methods including validated assessments, interviews, and direct observation of behaviour.

What do we need from you?

  • Social Worker or registered Psychologist essential
  • Strong experience and knowledge in dementia care and mental health for older persons
  • High level of communication and interpersonal skills to lead and influence
  • Excellent problem solving and decisionmaking skills
  • Ability to work independently and as part of a team
  • Excellent written and verbal communication skills
  • Prior experience working in Aged Care would be great, but is not essential
