The Role
The Support Coordinator will work as part of a fantastic team in either at home or at one of our multiple offices based in Laverton North, Sunshine, Broadmeadows and Kensington and bring with them sound NDIS knowledge.
They will help individuals build an ability to exercise choice and control, to coordinate supports and access their local community.
Responsibilities
- Support participants to understand and implement the funded supports available in their plan
- Link participants to community, mainstream and other government services
- Support participants to build skills as well as connect to relevant providers
- Ensure mainstream services meet the obligations to the participant (Housing, education, justice, health)
Key Competencies
- Genuine desire to provide the best quality support for people with disability
- Relevant tertiary qualification or certificate with relevant experience, or relevant extensive experience in case management, disability or community services
- Prior experience in NDIS Support Coordination
- Written, oral communication and interpersonal skills
- Creative thinking and an openness to new challenges
- A current Driver's License
- NDIS Worker Screening Check
- Proficiency in the use of standard office and client management systems

Salary:
$57, $91,813.00 per year
