About the role

In this role, you will be responsible for the provision on Care Management services to a case load of Home Care Package clients.

You will be involved in the development and review of care plans and have oversight of the allocated funds of clients who are recipients of Home Care Packages and their carers within our community.

Key responsibilities include:

  • Develop and maintain effective referral, assessment and review mechanisms and care plans to manage the needs of prospective and existing service recipients.
  • Implement care management practices which promote privacy and confidentiality.
  • Ensure appropriate planning, provision, coordination, integration, development and evaluation of Care Management Services.
  • Provide and maintain uptodate knowledge of developments in the Home Care Package Program and impacts on provision of care and services to clients
  • Effectively manage Home Care Package budgets ensuring that services and supports are managed within the limits of the package whilst also meeting clients' needs.

About you

To be successful in this role, you will be an experienced aged care case manager who enjoys engaging with a diverse range of people, and with the ability to build rapport with clients and stakeholders.

You will have a professional approach to your work, have high levels of resilience and the ability to be innovative and think outside the box when finding solutions to problems.


Essential Experience:

  • Experience managing issues relating to the provision of Home Care Packages supports and services to Older Australians
  • Awareness and support of the philosophies of healthy ageing, consumer directed care, promotion of independence including a collaborative approach to decision making.
  • Sound knowledge of the Home Care Package Program (HCPP) and the Commonwealth Home Support Program (CHSP).
  • Knowledge of government and community resources available for Older Australians
  • Current employee Working with Children Check & Victorian Drivers License
  • Tertiary qualification in Community Services, Aged Care, Allied Health or as a Registerd Nurse (Div 1 or 2) or demonstrated experience.
