Responsibilities
- Conduct comprehensive OT assessments for clients, including those with multiple and complex needs.
- Provide specific OT services and programs, including home assessments and advice on activities of daily living with recommendations involving home modifications equipment prescription care planning education Facilitate assist in development of a range of community based Health Wellness programs groups Involve clients carers in developing care plans that are person centred reablement focused Provide support education direction to community workers staff to assist them to best support our clients Submit individual applications for home modifications equipment accordance program guidelines Work collaboratively wider allied health team nurses physiotherapists fitness instructors allied health assistants Face face clinical work minimum travel time between clients time allocated administration reporting Ongoing liaison external agencies suppliers arrange services products required by clients accordance policies procedures
