Job summary
An exciting opportunity exists to join the established Disability Resource Team within South Western Sydney Local Health District as a Disability Resource Coach (Disability Navigator). In this role, you will make a meaningful difference by improving the care experiences of people with disability, their families and informal supports across SWSLHD facilities and services.- You will work collaboratively with multidisciplinary teams and external stakeholders to identify and address disability‑related needs, establish reasonable adjustments and support safe, coordinated discharge planning.
- Through proactive care navigation and hospital avoidance strategies, you will help strengthen continuity of care between admitted and community settings.
