About the role:


As the Support Coordinator, you will assist participants in implementing their NDIS funded plan and undertake preplanning and support coordination with participants, while taking a strengths-based and person-centered approach.


About you:


You have strong knowledge and experience in NDIS plans and funding categories, NDIS legislation, local community and service providers and with the ability to research, coordinate and manage a range of supports to suit individual needs across multiple providers.
