Service Overview:


Clarity Health Care is an accredited mental health service delivering a multidisciplinary, responsive program offering care to people with any form of mental health concern or mental illness.

Our mental health programs offer psychiatrists, psychologists, occupational therapists, social workers and registered nurses. More important than discipline, is the individual skill of the senior mental health clinicians we employ.

Evidence based care is delivered through outreach services, clinic based appointments, telehealth and group programs, based on clinical reasoning and need.

Due to the design of the workforce at Clarity, collaboration and continuity of care is core to the delivery of the treatment, with the recovery model and bio-psycho-social model being the grounding principles of all care.

Reporting Line:

This position reports to the Team Leader


Key Responsibilities

  • Provide an average of 6 client consults per 8 hour day.
  • Work in the intake/triage role, responding to service enquiries, and screening new referrals using the biopsychosocial and initial assessment tools
  • Deliver assertive, practical and evidencebased interventions to client's and their families as defined by the needs determined in the assessment
  • Advocate on behalf of clients with other clinicians, schools, employers etc. to enhance interventions and the wellbeing of the client
  • Deliver individual, group, and family therapy.
  • Undertake assessments as required based on the client's needs.
  • Develop, review, and close care plans in partnership with the clients and their identified supports
  • Assess, monitor, and manage the client's mental health and wellbeing (including physical health concerns) and risks; and collaborate with other health professionals, partner organisation and stakeholders as required
  • Work within the
    CHC framework of early intervention, assertive treatment, destigmatisation, diversity, inclusion, and nondiscrimination
  • Participate in multidisciplinary clinical and team meetings and case conferences that contribute to high quality outcomes for consumers and their families and carers as delegated by your manager
  • Participate in regular clinical supervision and line management discussions with your manager
  • Identify and take an active role in quality and service improvement activities.
  • Develop clinical practice knowledge and expertise through commitment to professional development and active learning within the team.
  • Plan and participate in training and secondary consultation as required
  • Provide mentorship and supervision to students at Clarity Health Care as required
  • Use IT systems including Microsoft Office and Electronic Medical Records, and maintain clinical files, records, and minimum data set requirements to facilitate good clinical management and accountability
  • Write reports as required for internal/external use i.e. assessment formulation, incident reports, funding body reports, court hearing.
  • Ensure billing is finalised each day
  • Other duties consistent with the role as required

Requirements:


  • Current AHPRA registration as a psychologist or provisionally registered psychologist, registered nurse, social worker or occupational therapist
  • Demonstrated experience in undertaking comprehensive mental health assessments and
development of intervention plans in collaboration with consumers and their family and carers

  • Demonstrated skills and experience in providing holistic, evidencebased therapeutic services
  • Understanding of the biological, psychological and social underpinnings of mental health
  • Demonstrated excellent interpersonal, communication and consultation skills.
  • Demonstrated ability to work as part of a team as well as independently.
  • Understanding of legislative frameworks relevant to mental health consumers and families and
- carers

  • Welldeveloped skills and knowledge of MS Office software and ability to use clinical
information systems

  • Demonstrated ability to collaborate, engage and negotiate with a diverse range of clients and other relevant individuals and agencies across sector.
  • Ability to foster, maintain and enhance partnerships/stakeholder relationships.
  • Ability to work autonomously under limited supervision, exercising sound professional judgement and seeking advice and consultation when appropriate
  • Demonstrated ability to work as part of a team
  • Understanding of legislative frameworks relevant to mental health consumers and families and
carers

$88, $116,584.00 per year
