The primary objectives of the Service Manager for Community Access and Complex Care Participants are to provide effective leadership and supervision of care teams, ensure the provision of high-quality, person-centred care to individuals with complex care needs in the community, and coordinate comprehensive care services.

This includes creating individualised care plans, optimising resource allocation, monitoring and improving service quality, and fostering collaboration among various stakeholders, all while adhering to regulatory and compliance requirements.

The Service Manager plays a pivotal role in enhancing the well-being and quality of life for the program's participants while maintaining a supportive and efficient work environment for the care team.

Key Responsibilities and Brief Description of Duties


Team Leadership and Supervision:


  • Lead and manage a team of care providers, including recruitment, training, and performance evaluation.
  • Foster a positive and inclusive work environment that promotes teamwork and professional development.

Participant Care Coordination:


  • Collaborate with healthcare professionals, social workers, and other stakeholders to create individualised care plans for complex care participants.
  • Ensure participants have access to a range of community resources and support services to meet their unique needs

Quality Assurance and Compliance:


  • Monitor and evaluate the quality of care provided, ensuring compliance with industry standards and regulatory requirements.
  • Implement and maintain policies and procedures to ensure the safety and wellbeing of participants.

Financial Management:


  • Monitor and evaluate the quality of care provided, ensuring compliance with industry standards and regulatory requirements.

Data and Documentation:

Salary:
$75, $95,000.00 per year
