_ The Role_


The Aboriginal Health Worker will work within the Integrated Care Service (ICS) based in the health centres to increase access to health treatment and prevention services for Aboriginal people and have a strong focus on the prevention and management of chronic disease with an emphasis on improving the patient journey from custody to community.


The Integrated Care Service aims to support and enhance the existing systems to provide an integrated model of care for adult patients in custody and on release into the community with chronic or complex health conditions.

Furthermore, ICS is to monitor patients from entry at reception through custody and post release unto the community, ensuring they receive seamless, timely, appropriate, and effective health care.
