We are currently seeking a dedicated NDIS Support Coordination Team Leader who is passionate about creating positive change and helping individuals overcome challenges.
Role Description:
As the NDIS Support Coordination Team Leader, your role is crucial in leading and managing the Support Coordinators/Psychosocial Recovery Coaches within the team.
You will oversee the daily operations of the NDIS Support Coordination Services by leading the team and also handle some billable work of participants directly.
Roles and Responsibilities:
- Lead and manage the daytoday operations of the NDIS Support Coordination Services.
- Contribute to participant assessment, care planning, and response to referrals in accordance with 9D Care's Policies and procedures.
- Support clients in developing, implementing, and monitoring goals using personcentered tools.
- Establish and maintain network relationships with key stakeholders, service providers, and referral services.
- Ensure accurate monitoring and reporting through meticulous client recordkeeping, adhering to confidentiality and privacy requirements.
- Address complex barriers impacting a participant's access to appropriate supports.
- Design service plans tailored to participants' specific support needs.
- Ensure participants are supported in exploring and fulfilling their needs, expectations, and goals.
- Collate Billable reports from the Team and send the Report to the claiming team on a regular basis.
- Manage the workload of the current Support Coordinators in a way that nobody is overloaded with new participants at anytime so that special care can be given towards quality of Support Coordination Services.
Criteria:
- Minimum Certificate III in disability services, aged care, or relevant qualifications, or Bachelor's in psychology, OT, or a relevant health field.
- Competency in working alongside individuals, families, and carers to build community participation capacity.
- Experience working with people with disabilities.
- Knowledge of NDIS Operational Guidelines and experience navigating NDIS systems.
- Understanding of case management practices and principles.
- Strong communication skills (both oral and written) with the ability to prepare reports and correspondence.
- Proficiency in using computer software programs and accurate data entry, including tracking funding for NDIS plans.
- Organizational skills, effective time management, and task prioritization.
- Ability to liaise with health and government systems with enthusiasm for working with NDIS participants.
Essential Criteria:
- NDIS Screening check.
- Driver's license and insured vehicle.
- Working with Children Check card.
- Double vaccination against COVID19 (Certificate required).
- Working with Children Check.

$75, $90,000.00 per year
