Case Manager - Aged Care

Your Role

As the Customer Support Manager each day will look a little different and your key responsibilities will include:

  • Conducting reviews for our clients, building, and revising their care plans, goals, services and expenditure
  • Establish Home Care Agreements and ensure all compliance and care governance documentation is completed accurately and correctly
  • Developing and maintaining excellent relationships with clients, client's family and advocates, staff, funding bodies, doctors and other allied healthcare professionals
  • Assisting consumers to make purchases through their Home Care package which are in line with their goals and government regulations
  • Making process improvement and implementing improvement requests as required
  • Contributing to growth of the Home Care Package program

Your experience in the Aged Care sector will enable you to:


  • Provide advice and build care plans
  • Manage funding requirements to ensure our clients can achieve the best care and support to brighten their lives.
  • Deliver high quality, client centred care
To do well in this role you will have:

  • Experience with care planning processes, including assessment and establishing client goals
  • A customer focus with excellent communication and problemsolving skills
  • Appropriate experience or qualifications in Nursing and /or, Community Services & Frontline Management
  • Ability to manage time effectively in an environment of changing priorities
  • Experience with Homecare & My Aged Care packages and funding requirements
