An exciting fixed term part time opportunity currently exists for an enthusiastic and highly experienced
CDAMS Clinician to join our
HIP Cognitive, Dementia and Memory Service (CDAMS).


Details of Department:


The Health Independence Program services aim to assist people with chronic disease, aged and/or complex needs that frequently use hospitals or are risk of hospitalisation.


The services of the HIP CDAMS are available to patients with cognitive and memory issues to remain living at home and to prevent unnecessary representation to the Emergency Department or admission to Hospital.

The service operates on Monday to Friday with the multi-disciplinary clinics taking place on Monday and Wednesday afternoons.

The HIP CDAMS has a strong commitment to professional development and education of staff.


The CDAMS Clinician employed in this generic keyworker position will be based at the Heidelberg Repatriation Campus of Austin Health.


To be successful in this role you must demonstrate:

Allied Health Professional or Registered Nurse or Medical Scientist, Pharmacist and Psychologist with relevant registration to your professional body

Broad experience with clients who have cognitive and memory impairment
:

Experience in coordination of service provision
:

Excellent interpersonal communication (written, verbal and computer)
:

Ability to work effectively in a team environment
:

Advanced casework skills, including conflict and crisis management, education, service linkage

Sensitivity to the needs of patients from culturally and linguistically diverse backgrounds

Well-developed workload and time management skills

Ability to problem solve in a variety of complex situations

A current Victorian Driver's Licence
