We have over 20years experience supporting people with physical, intellectual and multiple disabilities and developmental delays. Delta Community Support specialises in Supported Independent Living (SIL) and Community Access/Participation. Our team are passionate about enabling more people with disability to have greater choice and control.

We are growing and are looking for another Team Leader for our team in Aspley, Brisbane North. A mixture of 2days Office based, then the remainder working with Clients & Support workers on the floor.


Qualifications & experience

  • A minimum 4years experience in the Disability or Youth sector(s) in a Supervisory or Team Leader role. Qualifications at Certificate III level or above in; Individual Support, Community Services, or Social Sciences.
  • Extensive understanding/knowledge of the NDIS funding models and care
  • Current QLD Driver's Licence & own vehicle
  • NDIS Worker Screening & Blue Card

Tasks & responsibilities

  • Collaboratively establishing client care plans and overseeing all care provided from Support Workers
  • Providing direct support, supervision and guidance to participants to implement their personal goals and NDIS plan
  • Assisting with appointments, follow up activities and plans with Schedulers
  • Managing disabilityrelated behaviour or complex needs when necessary.
  • Interacting and communicating with other team members to ensure participants receive a consistent approach to service provision
  • Strengthening the abilities of participants and building their capacity to achieve their goals.
  • Cover emergency shifts or supports with clients where required
  • Prepare and collate reports as required by stakeholders
  • Assisting in the onboarding of new participants into the Organisation.

Salary:
$50, $65,000.00 per year
