About the Role
This is a Permanent, Full Time position with at 76 hours per fortnight within the Diabetes Allied Health Team.


You will work in conjunction with the Diabetes and Allied Health team, and be responsible for identifying and coordinating care and support for the most vulnerable patients within the Diabeties service at the Royal Childrems Hospital (RCH).

Thsi role will be involved in identifying and addressing psychosocial concerns, determining appropriate plans of support, referral to community bases services, and liaison with key organisations such as statutory child protection services.

Classification for this position will range from ZF- ZJ4, Clinical Nurse Consultant A - Clinicial Nurse Consultant B, (FTE base salary $107,962 to $113,646 per annum, plus superannuation)


What you'll achieve:


  • Providing evidencebased psychosocial support and referral for children, young people and their families diagnosed with diabetes of any type
  • Identify and address the psychosocial needs of children and young people newly diagnosed and/or identified by the multidisciplinary team as being vulnerable or at risk
  • Develop plans of care and support for vulnerable children and young people including referral to required internal and external supports
  • Act as an ongoing liaison as required with identified internal and external supports and key agencies
  • Coordinate and participate in multidisciplinary review of children and young people identified as vulnerable or at risk
  • Advocate for the needs of vulnerable children, young people and their families and carers, ensuring the provision of child centred, family focused care through partnership with parents/families/carers

Your skills and experience:


  • Current registration as a Registered Nurse
  • Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio (PPP)
  • Demonstrated capacity and skill in coordinating care for chronically ill patients and/or vulnerable children
  • Demonstrated capacity to work constructively and collaboratively with a range of disciplines towards common goals
  • Proven ability and skill to build collaborative relationships and work effectively within multidisciplinary teams
  • Post Graduate qualification or working towards

About the Diabetes Service


The Diabetes service at RCH cares for over 1500 children and adolescents with diabetes from metropolitan Melbourne and a further 300 children from regional Victoria with 2-5 new patients diagnosed per week.

The RCH diabetes service is involved in active primary care, education, family support, research and advocacy for children and adolescents with diabetes.

The Diabetes Allied Health Team is a key part of this service providing in-patient and ambulatory stabilisation of newly diagnosed patients, general inpatient and outpatient services and multidisciplinary outpatient clinics
