Position Summary


The Monash Health Community Access team is excited to announce the following position that is available for a Clinician (Allied Health Professional or Registered Nurse).

This position is permanent part time and presents an excellent opportunity for a like minded Clinician to join a dynamic team, that strives for excellence in Service delivery to our clients.


About Monash Health

Our aim is to empower and prepare clients to self-manage their health and health care.


Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non-acute care from prevention through to rehabilitation.

We provide
services that support clients in their homes, community based facilities and dedicated Monash Health Community hubs.


About the Role:


  • The role is integral to ensure that referred clients are triaged, registered and booked into an appointment with the most appropriate Service.

About You:


  • You are experienced, flexible and dynamic Nursing or Allied Health Professional that is looking for a change in career.
  • We are a high performing team and we ensure that our client's journey through Community Health is seamless and timely, it is therefore vital that we have a likeminded Clinician to continue this journey with us

You Are Also:

What You Need:


  • Relevant Qualifications in Allied Health or Nursing Related Field
  • Current Victorian Driver's Licence
  • Demonstrated experience working in a rehabilitation, aged care or community setting
  • Minimum 5 years post graduate experience
  • AHPRA registration or registration with National Registration Authority
  • COVID 19 Vaccination evidence
  • Australian working rights
