_About the Role_
The ACAT Assessor position stands within the CCH/ACAT multidisciplinary team.

The primary purpose of the position is to provide holistic comprehensive assessment, support and management to the target population in the St George Geographical area in the community, hospitals and aged care facilities as applicable.


_What you bring_

  • Qualifications in accordance with Sub Schedule C of the Named NSW (Non Declared) Affiliated Health Organisation's Professional and Associated Staff Agreement 2017 and eligibility for membership of AASW (Social Work),current APHRA Registration (Occupational Therapist/ Physiotherapist) and a current unrestricted Class C driver's licence.
  • Minimum of 3 years post graduate experience working with older people in an acute, aged care and/or community setting.
  • Knowledge of Aged Care Systems as well as Aged Care issues and experience working with clients from diverse cultural backgrounds.
  • Demonstrated ability to provide education to clients, peers and carers as well as commitment to ongoing selfeducation and best practice.
  • Demonstrated high level of interpersonal, written and verbal communication skills including computer literacy.
  • Demonstrated ability to work autonomously and as part of a multidisciplinary team.
  • Demonstrated knowledge of Quality Improvement initiatives and Work Health Safety principles relative to community service delivery.
  • Ability to work within the Mission and Values of Calvary Health Care Kogarah.
