An opportunity currently exists for an exerienced and passionate nursing or allied health professional to join our Community Aged Care Services team in a casual capacity

About the role

Working within the Community Aged Care Services Team, supported by the TCP Team Leader, with peer support from other case managers leader the role would see you:
Completing detailed individual assessment and care planning for eligible older clients of the Austin Health Transition Care or Restorative Care Programs focusing on the client's abilities, lifestyles and interests
Providing a period of case management, intensive service provision and / or therapy within their own home or aged care facility, enabling discharge to other community supports or to an aged care facility at optimal level of functioning
